IMPORTANT: You must have a transcript release signature on file at MHS to request a transcript online. If you are currently attending MHS, please see your guidance counselor to have a transcript request sent, DO NOT USE THIS FORM.
If you are nor sure whether or not you have a signature on file, email transcripts@mentorhigh.com providing your name, email address, and graduation year along with a request to check for a valid signature.
If you know that you do not have a signature on file, write a not giving your name, graduation year, and a statement that you would like transcripts sent as requested. Sign the note and mail it to:
