The Key to the Puzzle

The services you and your child received during the school years do not continue after graduation. If your child had an IEP during his school years, he may qualify for services as an adult. The key to receiving services from adult service providers is applying. Adults qualify for services based on the impact their disability has on their daily living. Disability categories are the same as those you know from the IEP process. But many people with mild or short-term disabilities do not qualify for services. The best place to begin looking for help is by contacting one of the primary service providers. These agencies can help consumers identify the types of services they need. They also have money to help pay for some of the services.

Crossroads provides outpatient mental health diagnosis and treatment, as well as outpatient chemical dependency services for adolescents and their families.

The agency's services include mental health consultation, community education, and prevention programs in elementary and secondary schools. Crossraods provides management, community support and "wraparound" services for children and their families.

The mission of the Lake County Board of MR/DD is to advance public awareness and to assist individuals with disabilities to become valued and contributing members of their chosen community. People with mental retardation and other life-long disabilities, including mental health needs may qualify for services through Deepwood. If you think you or someone in your family may qualify for services, you should contact Lake County Board of MR/DD directly to arrange an interview. They will help you with the application process. Deepwood (MR/DD) provides a variety of services for children and adults. Adult services include vocational, residential, and recreational assitance.

Waiting lists for services can be very long so it is wise to apply before graduation.

Neighboring provides mental health and community support services to mental health consumers and their families, including: counseling, employment and vocational rehabilitation services, consumer support groups, family support groups, and transportation to mental health appointments.

Pathways provides mental health services to Lake County residents age 18 and older. Community Support Workers assess client strengths and weaknesses, develop goals for improvement, provide ongoing support, and monitor treatment and mediaction. Either directly or through linkage with community resources, clients may receive help with transportation, housing, employment, education/training, medical services, and finances (Social Security, general assistance, and budgeting assistance). Other Pathways' services invlude the Senior Care Program, Homeless Outreach psychiatric counseling, medication management, and transitional housing.

Crisis intervention services are available to Lake County residents 24 hours a day, 7 days a week through Pre-hospital Screening, Community Based Stabilization, and the Lake County Crisis Hotline (440) 953-TALK.
