I am the Transition Coordinator for Mentor Schools. Transition is a big topic that covers all the activities that help a student move successfully from high school to the world after high school. Activities can include classroom instruction, career exploration, career assessment, career training programs, structured work experiences and a wide range of community based activities that expand daily living skills. Activities are planned to meet the needs of students’ individual education plans.
